Genetic variation and developmental disruption.
During development in the womb messages are sent out to the tissues to make one part bigger, one part longer, one part wider etc. It's the same system that results in stuff like the elephant's trunk, a message is sent out telling it to "grow, grow, grow".
Sometimes the messages bug out and an eye ends up situated lower on one side of the face, or a nostril ends up shorter than its sister. For the elephant its trunk might not grow at all and then you have a deformed elephant.
Nobody has a face that is truly symmetrical anyway.
