you can't suggest that they go to therapy, because they all absolutely refuse to accept that therapy might be an option

These conspiracies ring true for incels, because with their lack of self esteem and untreated mental illnesses, it offers the perfect excuse for their situation AND it absolves them of any responsibility. It isn't their fault they're lonely, women (worldwide) judge them for their negative canthal tilt/poor genes/height. They were just born unlucky. Not only that, but the "system" is stacked against them; sex isn't "evenly distributed" to them, because they aren't chad.
Okay, so now that our hypothetical incel has identified with these worldviews, he starts to feel different. Here's a brand new group of peers, all as unlucky in life as you are. This group is finally telling the truth about society. You've been tricked your whole life, and this realization makes it easier to swallow the other, more batshit ideas. If there is a global conspiracy from women to only give sex to chad, is it really such a stretch to believe that women want to be abused? They must like it. They only go for chads, and think of all the bad things that chads have done throughout history! They keep going back to them!
Their worldviews get further reinforced when people troll them. It is validating the idea that the world DOES hate them, because they are incel. Incels know the truth about female nature, and it must be true if the "Normies" are getting so upset. Normies have a vested interest in protecting the sexual market place so they can secure the distribution of women, right?
So really, it is the perfect storm for a young, inexperienced, and lonely man to latch onto if he thinks that life is unfair. Suddenly there's this group that not only agrees with you, but they have proof that your loneliness isn't your fault! The world is just stacked against incels!
The "us against the world" theme is what cements the group together. That subreddit is probably the only place they have felt like they belonged, and that people were listening to them. Would you really want to leave the only group of people who told you "it isn't your fault"?
I'm not going to talk about the mental illnesses they might have, because I've never even spoken to them IRL. But, I think that the environment draws in a certain type of person who has no self esteem, is lonely, and looking for answers. Add that to the fact that /r/incels encourages violent discussion about their crazy theories about women, and I'm sure that those thoughts become normalized very quickly.

It's something that takes a while to work, and you'll have to use it for the rest of your life (depending on how high functioning), but it could help you cross the social issue threshold (which is likely a large part of what is holding you back). Yeah it sucks, you have to work hard at something that comes naturally for the majority of the population, but it makes the chances of meeting someone who will take the time to know you more much higher.

At most some show signs of BDD, but I agree entirely with the points the first anon said, that they're nearly always exaggerating how bad their lives have been, they're not some underclass of society that people are constantly stomping on and mocking, no-one gives a shit about their virginity, even less at 19-20, the age most of them sit at.
They're just insecure people who can't bear to admit that they're at fault for their perceived failures, and so make up ridiculous theories and generalisations about everyone else, and blame them for their failures, attacking you and accusing you of being one of the people blamed for it (for example, a major part of their ideas about women is that women don't know what they want, or will lie to them and really just want chad, which excludes testimony from any women from being able to challenge their worldviews).
